Incident reporting

Have you been involved in an incident at work or noticed something that could pose a risk? If so, you must report it.
Here you will find guidance on how to proceed in different types of incidents at Stockholm University.

An incident is an unwanted event that has had, or could have, negative consequences. It may also refer to deficiencies that have not yet led to consequences, such as risks in the work environment. Other terms used include near miss, deviation or injury.

In emergency situations

If the situation is urgent, raise the alarm immediately.

Crisis and incident management

An incident is an event that affects or could affect the operations, for example an IT breach, an accident or a theft. Incidents may be both major and minor. It is essential that accidents, near misses and other incidents are reported, even if they seem less serious. This allows the matter to be addressed and reduces the risk of something similar happening again.

A crisis is a more serious event requiring rapid and coordinated action to protect people, operations and the University’s assets. In the longer term, it is also about safeguarding the University’s reputation. In the event of a crisis, the University’s local and/or central crisis management team is activated in accordance with the crisis management plan.

Report incidents

Here you will find different types of incidents and information on how they are handled at Stockholm University.

Report IT incidents and other events relating to information security as soon as possible. This may include suspected breaches, lost or stolen equipment, incorrect access rights or other events that may affect the security of information.

Your report will be handled by the Information Security function.

Information security (information in Swedish)

 Report a personal data breach if personal data has been lost, altered, disclosed or accessed by unauthorised persons – whether by accident or intentionally. This may include emails sent to the wrong recipient, lost documents or unauthorised access to systems.

Your report will be handled by the Data Protection Officer.

Manage a personal data breach (in Swedish)

Report in the IA system if you have been injured at work, become ill as a result of your work, or identified risks or deficiencies in the work environment. This also applies to near misses and accidents on the way to or from work. Your report will be handled by your immediate manager, Head of Department or equivalent.

Occupational injuries and incidents

An environmental non-conformity is an incident or condition that contravenes environmental legislation, local rules and procedures, or an environmental management system, and that has caused or may cause adverse environmental impact. Examples include chemical spills, improper energy use, unnecessary transport, or inadequate waste management. Your case will be handled by the central environmental function.

It is important that you report thefts and security observations in order to identify both positive security measures and potential risks.
