Checklist – employee or student accident or death on campus

What is important to do if a staff member or student has an accident or dies on campus?

Immediate actions

  1. Take action to prevent further injury.
  2. Call 112 and request an ambulance and the police.
  3. Inform the Head of Security, the Head of Department (equivalent) and the Dean. The Head of Security then contacts the others involved.
  4. The Head of Communications informs the switchboard that only the Head of Department and the Head of Communications will answer questions from the media.
  5. Ensure that someone calls the staff and/or students concerned for information as soon as possible.

Same day

  1. Gather relevant staff and/or students and inform them about what happened. Assess and offer assistance/support to the persons concerned. These will be channelled through Human Resources Office or Student Health Services. Listen actively and be sensitive to the different needs of individuals.
  2. Information to relatives is always normally provided by the police or the doctor who diagnosed the death. NOTE! Death notices should never be given by employees!
  3. Review the work situation and assess what measures need to be taken initially to ensure that the organisation can function satisfactorily.
  4. If the death is caused by an accident, inform the Swedish Work Environment Authority in Stockholm. This applies to both students and employees. You should also make a notification in the IA system.

Upcoming days

  1. Provide the same information to the work and student community as you have received yourself to avoid rumours or untrue information. Remain alert to the special support needs of individual staff and students. Contact Human Resources Office or Student Health Services if further support is needed.
  2. Listen to whether there are any special questions about the incident and what wishes there are for honouring the deceased.
  3. If there are no special requests from the family, the deceased is honoured at the funeral with a wreath with ribbons in the colours of Stockholm University. You as a manager will organise the ordering and delivery of the wreath. You should also attend the funeral ceremony. Who else(s) attends can only be decided by each individual unless the relatives have declined condolences.
  4. You as a manager may also have to ask a relative to come to the workplace to take care of the deceased's personal belongings.
  5. The Head of Department/Dean (or equivalent) decides whether the deceased should be honoured with a memorial service.
  6. Contact the IT Services to have the employee's personal data removed from the e-mail function and SUKAT.

These actions should be taken by the immediate manager, unless otherwise specified. If a manager is not immediately available, it is important that employees know what needs to be immediately done. Guidelines should therefore be made available in an appropriate place and staff and students informed of their content.
